About this scholarship
Minnesota or Iowa residents ages 16 to 27 who are the natural, adopted, step, or full dependent child of a worker injured or killed in a work-related accident with a Minnesota- or Iowa-based employer may apply. You must be a U.S. citizen or permanent resident with a high school diploma or GED (or be a high school student in good standing), demonstrate financial need, and pursue a bachelor's, associate's, vocational certificate, license, or master's degree at an accredited school.
The SFM Foundation awards scholarships of $15,000 to support students affected by a parent's work-related injury, regardless of the insurer involved.
Submit a complete application through the foundation. Awards are granted on a competitive basis with the number of recipients varying each cycle.

- Completed signed SFM Foundation Scholarship application
- Certified high-school transcript and, if applicable, certified college transcript
- Recent school ID or valid state driver's license/ID
- Two original non-relative letters of recommendation
- Documentation of parent or legal guardian injury or death, as applicable
- First Report of Injury if a workers' compensation claim was filed
- Verification of college acceptance and/or attendance
- Estimated education costs
- One-page letter explaining educational goals and financial need
- Completed and signed SFM Foundation Scholarship application form
- Certified high school transcript (must include grades and attendance); if previously/currently attending college, the previous semester's certified college transcript
- Copy of recent school ID card or valid state driver's license/ID card
- Two original letters of recommendation (must not be from a relative)
- Brief description of nature and extent of parent or legal guardian's injury (in Section II A or on an attached sheet)
- Certified death certificate of parent or legal guardian (if applicable)
- First Report of Injury (if a workers' comp claim was filed)
- Verification of college acceptance and/or past attendance
- Estimated costs: registration, tuition, textbooks, and living expenses
