Guelph Student Profile Form
Guelph's optional Student Profile Form lets high-school applicants describe leadership, citizenship, activities, and circumstances that affected grades. It is considered in the final offer round only when an average falls within a discretionary range of the program cutoff, at most 5%; it does not guarantee admission and does not replace a mandatory program form.

What to prepare
- 1Choose an activity, role, or responsibility and explain what you did and how it prepared you for university life.
- 2If circumstances beyond your control affected your grades, state the context and its academic effect plainly rather than relying on vague hardship language.
- 3Use examples that show growth or contribution, including family or community responsibilities when relevant; the form is an opportunity for context, not a substitute transcript.
How to approach the writing
- Check the live form for its actual fields and limits before drafting; Guelph's public explanation does not supply a fixed essay prompt.
- Keep the SPF separate from the disability-related Supplemental Information Form and from any mandatory program supplement such as the BLA form.
Before you submit
The published SPF deadline is April 1, 2027. Guelph says no third-party documentation is required and that the form is used for admission only, not to fulfill offer conditions.
