High school seniors living with a chronic disorder who are graduating in the award year and have been accepted to an accredited college or university may apply for Lawrence Madeiros Scholarship funding. The award amount varies.

The digital application requires identifying information, proof of the disorder, a transcript, a personal journey essay, and short answers on accomplishments, passions and goals, and scholarship impact.
