About this scholarship

Wisconsin high school senior athletes with at least a 3.5 GPA who participate in WIAA-sanctioned sports and community service may apply for the Driven at Heart Scholarship Contest from the Cousins Subs Make It Better Foundation.

Four scholarships are awarded: $7,500 each to one male and one female first-place winner, and $2,500 each to one male and one female runner-up. Four finalists attend the Wisconsin Athletic Hall of Fame Induction Ceremony.

Submit the online application by February 28. Finalists must be available to attend the ceremony in Milwaukee. The contest, presented with Donald Driver, recognizes athletes who excel in academics, sports leadership, and community involvement.

  • Online application.
  • Official transcript.
  • Two letters of recommendation: an athletic endorsement from school staff and a community-work endorsement from a community member.
  • Required essay responses.
  • Must be a second semester high school senior, college-bound, with cumulative GPA of at least 3.5 (4.0 scale)
  • Must have participated in a WIAA-sanctioned sport during high school career
  • Must be involved with civic/community organizations or activities during grades 9-12
  • Submit an official copy of transcripts
  • Submit two letters of recommendation: (1) athletic endorsement from a school staff member, (2) community endorsement for community work from a community member
  • Essays may be attached as a separate Word or PDF document if preferred
  • Must consent to be photographed/audio/video recorded; finalists sign a Publicity Release Form
  • Finalists must be available to attend the Wisconsin Athletic Hall of Fame Induction Ceremony in Milwaukee.
  • Finalists must consent to photography/audio/video recording and sign a publicity release form.
